Primary Responsibilities: The Client Case Manager is responsible for coordinating and facilitating the highest quality professional services to our clients who are participants in the State of Alaska Personal Care Services Program, the Medicaid Home and Community Based Waiver Services Program and our Private Duty services.

Position is: Hourly, Full-Time Reports to: Program Director

Salary Grade: $21.00-$29.00


Client Case Manager Accountability:

1. Providing oversight of PCA Service Plans, Waiver clients' Plan of Care, Private Duty Care Plans, and Family Habilitation Homes.

2. Ensuring oversight of Service Authorization for all 4 services; ensuring that clients have a current SA on file, and that it is updated when required. May include follow up with client, care coordinator and or State point of contact.

3. Conducting service inquiries, initial and renewal intakes, gathering signatures as needed in person via a home visit.

4. Maintaining a complete client record, which includes creating accurate and timely documentation of contact with clients and caregivers.

5. Acting as a liaison with clients' care coordinators.

6. Implementing quality improvement activities, which may include reeducation of client and/or caregivers to ensure compliance with Trinion, State, and Federal regulations, standards, and/or best practices.

7. Perform Quality Assurance home checks as scheduled or requested, at times in response to a concern or complaint triggering an internal QA investigation.

8. Oversight of internal Incident Reports. Filing of external Critical Incident Reports.

9. Responding to Adult Protective Services' inquiries about a client.

10. Maintaining all client records as required to demonstrate and document compliance with all regulatory, health and safety requirements.

11. Attending association meetings, webinars, and trainings provided by Trinion and other entities as needed and appropriate to essential job functions and professional development.

12. Maintain knowledge of current regulations and compliance.

13. Ensure HIPAA compliance in all aspects of Service Delivery.

14. Other duties as assigned.


Client Case Manager Authority:

1. Developing and maintaining relationships in clients' care and delivery of service.

2. Representing Trinion in the role of Client Case Manager.

3. Developing Care Plans for clients.


Position Specific Standards:

  • Must have the ability to communicate effectively verbally and in writing to both employees and clients with varying levels of communication skills.
  • Must possess the skills needed for creative thinking and problem solving.
  • Ability to work with various organizations within the industry of in-home health care, Medicaid, Insurance, and Private Payers.
  • The ability to coordinate multiple tasks efficiently while managing various and continuous interruptions.

6. Employees must be able to work effectively in a fast-paced environment with minimal direction and supervision while prioritizing conflicting priorities.

7. Knowledge of standard office equipment and systems (i.e. email, voicemail, fax) for all staff is preferred.

Minimum Education Requirements:

Bachelor's degree in Psychology, Social Work, or a related Social Sciences field

Training and/or Experience:

Must have two years of full-time or equivalent part-time experience working with human services recipients.

Preferred minimum 1 years' experience with Sales or Marketing.

Preferred minimum of 2 years' experience with Medicaid funded programs.

Work Conditions: Follows office work week 8 hrs/day and 40 hrs/week. Must be able to lift at least 25 lbs.
